Welcome to registration for the fall 2024 fall tackle season!
 

Checklist to complete registration 

1. Fill out all registration, medical and insurance questions.

2. Upload a copy child’s birth certificate ( only if this is your child’s first year in our program).

3. Download the Medical form, and have it completed by a licensed medical professional. Must be dated this calendar year.

4. Download the Insurance waiver. It must be filled out and signed.

5. Download the equipment waiver, it must be signed by a parent.

6. Medical forms pages 1 & 2 , if completed by a physician and dated this year and signed should be uploaded during registration. The insurance waiver and equipment waiver must be filled out, signed, and uploaded during registration.

7.  If you must have a new medical form prior to the start of school, please have it filled out and the send the form to the P.O. Box listed below.

8. Return completed  medical forms to our P.O. Box: Young Warriors Football Association P.O. Box 7011 Deerfield Illinois 60015.  All forms are due by NO later than August 5th, 2024

9.  All forms must be completed and the registration fee payed before a player will be eligible to participate in practices or games.  After you complete the registration process we will receive a registration entry number, and you will receive a confirmation email from SportsEngine.
